He trained in medicine at the University of the Witwatersrand in Johannesburg, came to Johns Hopkins for his psychiatry residency, and earned his MPH at the Johns Hopkins School of Hygiene and Public Health in 1982.

He began his research career studying personality disorders and obsessions and compulsions in the Baltimore community, as part of the NIMH Epidemiologic Catchment Area program. He went on to lead the Johns Hopkins OCD Family Study and two multisite, NIH-funded genetic studies of obsessive-compulsive disorder. More recently his group has looked at rare genetic variants in OCD, at doubt as a feature of the disorder, and at how OCD starts or worsens during pregnancy and after childbirth.

He has more than 250 publications, has treated patients with OCD at Johns Hopkins for more than thirty years, and is a Distinguished Fellow of the American Psychiatric Association.
